  • Application form from the Treasurer’s office
  • Sanitary/Health Permit from the DOH
  • Zoning Certification from the MPDO
  • Building Permit
  • BFP Clearance
  • SSS/DTI/Phil Health Certificate
  • BIR Clearance
  • Payment of Registration Fee - P500.00
  • PCAB License (if contractor engaged in construction industry)
  • NFA Clearance (if engaged in rice retail and Buy and Sell of agricultural products)
  • Food and Drug Administration (FDA) Clearance (if engaged as drug and medicine dealer)
  • PNP Clearance (if engaged in security agency services)
  • DENR Clearance (ECC/CNC) (if engaged in mining exploration)
  • Fertilizer and Pesticide Authority Permit (if engaged in dealership of agricultural supply)
