CSOs and PSOs seeking accreditation shall submit the following:

1. Letter of Application
2. Duly accomplished Application Form
3. Board Resolution signifying intention for accreditation for the purpose of membership in the local special bodies
4. Certification of Registration
5. List of current officers
6. Annual Accomplishment Report for the immediately preceding year
7. Financial Statement, also of the immediately preceding year
